PART 2: LIABILITY WAIVER AND RELEASE OF CLAIMS:

I have answered all questions above truthfully. I, the undersigned participant, voluntarily choose to receive a body composition scan and participate in physical movements related to a fitness assessment provided by Hamaca Fitness, LLC at this corporate wellness event. I understand that the Hume Body Pod Scale provides an **estimate** of body composition and that any physical assessments are for informational and educational purposes only. Neither is a substitute for medical examination, diagnosis, or treatment. I acknowledge that Hamaca Fitness, LLC is **not** medical professionals, registered dietitians, or physicians, and cannot diagnose medical conditions or offer medical advice based on the results. I agree to consult my doctor regarding the results and before making any changes to my routine. I understand that while BIA is safe, there is an inherent risk involved with any physical assessment or activity, including but not limited to, muscle strains and falls. I voluntarily assume all risks. In consideration of being permitted to receive the body scan, I hereby release, indemnify, hold harmless, and forever discharge Hamaca Fitness, LLC and its representatives from any and all liabilities, claims, or damages, **including but not limited to any claims based on negligence**, to the fullest extent permitted by law. I affirm that I am at least eighteen (18) years of age and competent to sign this release.
